1660 Referee Feedback Form


 

  • Please complete as much detail on this form so that we can properly address and follow up any issues or concerns, and ensue the directed to the proper administrators and officials.
  • If further details are required or additional documents are included, please note this in the comments section and attach them with an email to admin@1660.ab.ca.
  • Include ALL RELEVANT INFORMATION regarding the compliment or complaint as possible be sure to note where applicable:
            a) ALL events that may have led up to any specific incident(s), AND ALL events which may have occurred after the         incident(s)
            b) A DETAILED, CLEAR & CONCISE EXPLANATION of the incident so that a person not at the game would understand!
            c) Note and highlight the specific question or issue you would like addressed.

1660 Referee Comment and Feedback Form was designed so the league can forward the positive feedback to our referees to assist in the recruiting, development and retention of our referees. When a concern about a referee is received, it is reviewed, and if deemed it will be forwarded on through the proper channels. 

For a concern to be forwarded it needs to follow the proper steps: (North Central Referee Committee will no longer accept letters directly from parents, spectators, team officials, or players.  If you feel your concern is warranted, it must be presented through your team management, then submitted to the 1660 Hockey League, Local Minor Hockey Association, or  Hockey Alberta Zone Rep.

If the league, Local Minor Hockey Board, or Hockey Alberta Zone Rep. feels there is merit to the concern, they will forward the letter to either the Zone Discipline Chairman or the Zone Chairman. An investigation into the concern will be conducted by the Referee Council. Then a response to the league, Local Minor Hockey Association, or Hockey Alberta Zone Rep and team manager.

COMPLAINTS CAN ONLY BE COMPLETED A MINIMUM OF 24 HOURS AFTER THE COMPLETION OF THE GAME, Compliments are welcome at anytime!
